Risk Assessment for filming at various locations

Today I have been filling out a risk assessment form, which shows all the hazards, the people that are likely to be harmed while filming, how severe could the harm be, the different kinds of controls we are going to have at different locations, how likely the accidents are going to occur and it also includes a risk rating, which in our case is pretty low, as we are going to be organised and will be carrying a first aid kit and a mobile phone for emergency purposes.
